Faith Care Connection

Support for Congregants

The Faith Care Connection program seeks to lend support to vulnerable individuals in the community who are living alone and at risk for cognitive changes or other changes due to aging. Our approach is unique in that we are identifying those at risk through church connections.

One in three people over the age of 85 have dementia, yet more than half have not been diagnosed.

Working alongside pastoral care/senior ministers in churches, we have a licensed professional reach out to those individuals identified to be at risk for cognitive changes. Our role is to assess social isolation, depression and quality of life; and provide recommendations, referrals and follow-ups with their care over the course of a year.
